Here is what I'm using.  In /etc/rc.conf:

  moused_enable="YES"
  moused_flags="-m2=4 -m4=2"

And in /etc/X11/XF86Config:

  Section "InputDevice"
        Identifier  "Mouse0"
        Driver      "mouse"
        Option      "Protocol" "Auto"
        Option      "Device" "/dev/sysmouse"
        Option      "Resolution" "250"
        Option      "EmulateWheel"
        Option      "EmulateWheelInertia" "60"
        Option      "EmulateWheelButton" "4"
        Option      "Buttons" "4"
  EndSection

This gives me 3 buttons, plus an emulated "scroll wheel", when I hold
down the fourth button and move my finger up or down on the mousepad.
(see mouse(4) in /usr/X11R6/man, not mouse(4) in /usr/share/man...)
