Log:
  MFC r196837:
  Remove artificial MAX_IO_SIZE constant, equal to DFLTPHYS * 2. Use MAXPHYS
  instead. It is NULL change for GENERIC kernel, but allows 'fast' mode to
  work on systems with increased MAXPHYS.
