Actually, what has happened is that the end of the tape has twisted 90 deg. and lies "on top of the casserole" close to the tape rotation axis. (Hey, my English isn't native, please *try* to
understand ;-)

It is more likely that it has been rewinded too fast than that it has gotten stuck in the capstan or around the tape heads.
