1. Have you checked whether or not qmail is login in /var/log? I mean permission denied could mean that it didn't start, but also that it had problems doing (accessing) something. 2. Are you sure the /var file system is mounted with the proper privelages? 3. Are you sure the kern.securelevel of your system is correct?
