Here is one that's not on your list: http://www.sisd.com/freeside/
It's perl rather than php, but it does use Apache and MySQL. It 
pushes data to a Radius users file (or you can use a sql radius like
Radiator, that one isn't free but is very flexible with it's
accounting and access control, you can use sql queries to determine
if a user is allowed in), erpcd, passwd/master.passwd, nis, ...
also does qmail and shell accounts. I don't think Freeside does 
radius accounting itself, you would need something else for that.

Maybe bb (big brother)? I haven't used it though so I don't
know what it's like. If you want something more in depth there
is a lot you can do with scotty.
