That is normal. H323 can only send video and audio between
boxes with real IP addresses.

Unless you use a H323 Proxy which runs on a machine
which can see both the real internet and your
internal private network. (ie your FreeBSD box with NAT)

So you need to use the openh323proxy port which was
added to the FreeBSD ports tree about 4 days ago.
