I was running ctived also...

In any case, I set up an new box, installed the same software, moved my
spool drives over.   The machine has been up 4 days with no problems
whatsoever.

Therefore, I need to report a bug:

Upgrading FreeBSD from 2.1.6 to 2.2.2 using the install floppy breaks
MMAP.
