We already have following chunk in moused(8):

     -3      Emulate the third (middle) button for 2-button mice. It is emu-
             lated by pressing the left and right physical buttons simultane-
             ously.

Isn't this code that you want to have by adding SC_TWOBUTTON_MOUSE ?
