This should happen automatically, when an MTA uses the DNSBL feature.
I don't know about sendmail, but with postfix, all you need to do
is to add the following lines to the smtpd_recipient_restrictions
field in /usr/local/etc/postfix/main.cf:

smtpd_recipient_restrictions =
  # ..... more restrictions here .....
  reject_rbl_client bl.spamcop.net,
  reject_rbl_client sbl-xbl.spamhaus.org,
  reject_rbl_client list.dsbl.org,
  reject_rbl_client dnsbl.njabl.org,
  reject_rbl_client relays.ordb.org,
  reject_rbl_client opm.blitzed.org,
  reject_rbl_client dnsbl.sorbs.net,
  # ..... more restrictions here .....

The descriptive text, if provided by the blacklist as a TXT DNS
record, is then always included in the bounce message.
