Try to storage the default:

	# rm -rf ~/.local/share/applications
	# rm -f ~/.config/menus/applications.menu

There is a known issue in smeg that cause menu disappear in the previous  
version, but not in up to date smeg and py-xdg.. I just checked in Ubuntu  
forums.

gnome-menu-editor still isn't complete done. It's kind of shame on GNOME  
team for not finish menu editor. :-/

Same place, right click on file -> properties -> 'Open With' then change  
the default app what you want to open. Or right click on file -> 'Open  
with [...]' and select what apps you want to open.

Maybe need to select on eog then delete gimp? I can see that it will not  
allow us to remove if it only has a entry. It's something that you will  
have to report in GNOME bugzilla.

Yes it does, maybe need to clear up somewhere in ~/.* configure file.. Try  
to login in a dummy account, then use Epiphany to find PDF link and click  
on it.
