We had similar issues with MySQL 4.1.20 on FreeBSD 4.11-STABLE today.
Compilation of mysqld with debug information revealed some isssues with
mysql startup in BDB initialization. The issue was resolved by
uninstalling db41 port, installing db42 and reinstalling 4.1.20.
