just to answer my own question: after many, many hours of experimentation: i don't know how many other dot matrix printers this may apply to, but for fbsd 2.2.8 + gs 5.10 + an epson fx 100: for gs, set r240x144 and sDEVICE=okiibm. none of the epson drivers worked well, and no other resolution works well with "okiibm". thanks.
