This is going to sound funny, but have you tried unplugging all of
your other phone equipment?  When I first set up an old USR 28.8
PCMCIA modem I found that it would return NO DIALTONE if I had a
phone off the hook (to listen to what was happening on the line).
I don't know whether it was an electrical issue or the extra audio
from the phone, but it was consistent.
