Hi,

With a liberal sprinkling of null content .Bd + .Ed pairings
I was able to get the section headers to behave, but there
are still some minor glitches with lists.

You may also be interested in my manual page writeup at:

http://sixshooter.v6.thrupoint.net/manpg/book.html

although there is no mention of groff -Thtml
(and not likely to be one after this exercise!)

Try the patch below and see if it helps.
