Both apply as a verb, but "make" is the only one I've seen used as a
noun.

No.

Ok, portupgrade will do the complete install process. If you can
create a command to apply your local patches - and it's the same for
each port - you can use the -B flag to make to get it to run that
command. I can see a straightforward script that looks for patches in
/usr/ports-local/<port-specific-dir> and applies them if they exist.
