Also if its PCI it may be a winmodem and you may be out of luck.

There IS  a Lucent Chipset winmodem driver that may work for you but you
would have to find someone who knows where it is or check the archives to
find it for sure.

I have used the Linux version of this driver with a great deal of success
under 2.2.12 <note that I had to have a very specific kernel in order to
use the driver in linux>
