The Microsoft compatible mice do not have a standardized 3-button 
protocol, therefore X treats it as a 2-button mouse.  You can put 
"Emulate3Buttons" in the "Pointer" section of your /etc/XF86Config.  Then 
you can "press" the middle button by SIMULTANEOUSLY pressing the leftmost 
and rightmost buttons.
