The problem is, that the interrupt request from the card does not arrive
at the driver interrupt routine.

This and possible solutions are described in the FAQ and the mailing list
archives.

It might be, that the assignment to IRQ 5 is the problem since IRQ 5 is
usually assigned to a parallel printer port. Check your BIOS settings!
