Going from 3.4 to 4.0 is quite tricky. You should not be doing it for
the first time on a production machine. It is questionable why you
would be running 4.0 on a production machine at all.

3.4 should be pretty much the same as other 3.x's.

Dunno what that is. But do format the floppies again yourself before
trying again. "Factory formatted" floppies have been known to be
trouble.
