The information so far is a good start; it's a disk configured with 
'disklabel auto'.  Now, how about the failure mode?  What version of 
kern/vfs_conf.c are you using?  How old is the bootblock on the disk?

I replied to you just after I believed I had corrected the behaviour 
for cases such as yours.
