Actually, the form-feed isn't the problem. I tried printing a 3 page
document where the 3rd page was empty and it stopped in the same
place. Halfway through printing out the 2nd page. A 1 page doc prints
great.

I copied and pasted the script at

Yes, I left the default lp in there and put the above lines right below
it.

I just noticed about adding an err log file. No errors printed to it and
it stopped again halfway through the second page.
