Have you tried going to SMC's site and getting the software that will 
allow you to reflash the rom on the SMC EZ card?

If you do this, I would suggest IRQ: 10 & I/O 300   or IRQ: 11 & I/O 320
For some reason these seem to work with the least hassle for ISA cards.
