How much work would it take to tweak the CDROM driver to wait 5-15 minutes
(or, perhaps add an option somewhere, to make it dynamically configurable)
before spinning down? I think that would solve some of the problem.

Of course, you'd probably need cooling if you're going to have a 32x at
full spin all the time.
