Thanks, I'll take a look at it. I don't have the hardware to test
though, so I don't know at this time how fruitful it will be that
I look at it.

A failed format attempt may render the disk useless. The I/O error
may not be related to the DMA problem that I fixed, but to a bad
floppy as the result of the broken fdformat.
