It's bad hardware, very clearly in your case (since turning off the
external cache worked).  You don't describe your disk controller, but
if it's a SCSI busmaster controller, it might help to enable the
external cache but operate it in `write-through' mode.  This doesn't
lose much performance (unlike turning it off totally).
