I have very same failures after upgrading from 11-STABLE to 13-STABLE, or f= rom 12-STABLE to 13-STABLE. My systems are not busy and the amount of used swap is tiny. Just after OS reboots, pf is working as expected. But they tend to fail when their runtime getting longer.
