I've never liked the idea of starting it from /etc/ttys.  I start it
from a script in /usr/X11R6/etc/rc.d.

I use my own xdm-config file which does specify that the pid file goes
in /var/run.  If booting without removing it is a problem, you should
remove it before starting xdm at boot time.  I've never noticed such
a problem myself.
