Please consider print output when making this kind of change.  This
now prevents the link from working in PDF files, and it makes the link
meaningless on paper or PalmPilots.

It is best if entities can be used so that relative or absolute URLs
can be built in the document as appropriate.  Failing that, I prefer
absolute URLs.
