Ok, the handbook is wrong - I'll fix it ASAP.

Yup, if a number is skipped, then no following aliases will be configured.
Check the manpage for rc.conf for more detail.

No, that's correct - if the alias is on the same subnet as an already
configured address, then the subnet mask should be 255.255.255.255.

It's because they're not supposed to.  Read the rc.conf manpage, specifically
the "network_interfaces" bit.
