Yay timing issues. Your drive is jumpered for master and/or single
operation, yes? have you replaced the cable?

Use a serial console to capture the output. String up another machine via
a null-modem cable to COM1, start up a comm program, set it to 9600 baud.
Then boot the CD, drop to the prompt at the loader menu, enter the command
'set console=comconsole'.  You should get an OK prompt on the serial
console. Turn on capture then 'boot -v' at the serial console.

Judging from the output, it looks like IRQ 11 is getting misrouted.
