Make sure these are correct.

Are you using -auto or -ddial?  If not you can remove this line.

>  add 0 0 HISADDR

These belong in /etc/ppp/ppp.linkup.

Try upgrading your version of ppp to the one at
http://www.freebsd.org/~brian.  Lots of changes have happened since 2.2.2
(primary Brian's takeover of the code).
