BTW, does out system ftpd calls opieaccess()? As I look through the code,
it is not (but maybe it burried somewhere inside PAM, I am not sure).

If not, it is the bug indeed making tunneling not possible even with
INSECURE_OPIE. To fix it, please examine the way wu-ftpd does that. I am
familiar with regular OPIE and know how to fix things in that area, but
know nothing about PAM.
