I had the same problem when setting up HylaFAX and trying to put
faxgetty in /etc/ttys.  I ended up finally not messing with ttys and
putting it in rc.local along with faxq, but i tried everything you did..
Remove the line, HUP init, kill faxgetty... HUP init AGAIN.. kill
faxgetty AGAIN... all to no avail.  Finally rebooted and it never popped
up again.
