I installed three new installations of FreeBSD 4.1.1 in the last 8 days.
All three systems entirely different.  One system with a Logitech 3
button mouse, one with a Microsoft 2 button and another off brand 2
button mouse.  All three machines have seen the psmintr: out of sync
error.  Each machine needed to be rebooted to alleviate the problem.
One thing in common, the Enlightenment window manager was being used on
all three systems.   I also noticed that one of the machines displayed
the behavior shortly after increasing the mouse sensitivity via the KDE
control panel in Enlightenment.  The default kernel was in place during
these errors except for the Athon system.

Machine 1: PIII 500 - Compaq
Machine 2: Athon 800
Machine 3. PIII 300 - Compaq
