I'll bet you haven't looked at what's in /var/log/messages.  You'll
almost certainly find ioctl conflicts there, indicating that your
vinum(8) is out of sync with the kernel.  In any case, check
http://www.vinumvm.org/vinum/how-to-debug.html and do what it suggests
there.
