Add the ability to use ATAPI devices via CAM.

The CAM<>ATAPI layer was submitted by "Thomas Quinot
<thomas@cuivre.fr.eu.org>"
changes form the version on the net by me (formatting, ability to be used
alone without the ATAPI native device driver, proper speed reporting...)

See /sys/conf/NOTES for usage.
