FWIW, I've had this same problem with -CURRENT for a long time (since
maybe Septemer of last year, give or take, I'm not exactly sure).  It
lags several seconds with Realplayer, does the same with mtv[p] (gr, need
a good MPEG A/V player), BUT stays nicely in sync with .avi's through
xanim, and doesn't have near as much lag with XMMS (still .5-1 sec
though).
