IRQ 7 is a poor choice.  That's the default printer IRQ and I
think that even disabling lpt0 doesn't change that.  Try IRQ 5
which is also the default kernel setting for COM3

You don't, use /dev/cuaa2

There are several cuaaX's and ttydXs too.  The cuaa ports are the
callout ports, probably what you want.  man sio(4)

I don't see those options in my kernel config file

device          sio0    at isa? port "IO_COM1" tty irq 4 vector siointr
device          sio1    at isa? port "IO_COM2" tty irq 3 vector siointr
device          sio2    at isa? disable port "IO_COM3" tty irq 5 vector siointr
device          sio3    at isa? disable port "IO_COM4" tty irq 9 vector siointr
