Yep,  That was the error, thanks.  
Funny, I reformatted the floppy, and ran a disk check
on it and it still tested OK, but wouldn't work.

I put the image on a new floppy, and it worked fine.

The FAQ doesn't cover this, exactly.  However, it means that track 6
of your floppy is bad.  Try another floppy.
