xc/doc/specs/xterm/ctlseqs.ms in the X11 sources contains the
specification for the control sequences.  You can find a PS compiled
version as xc/doc/hardcopy/xterm/ctlseqs.PS.Z in the distribution.

If you fail to find this somewhere, drop me a mail.  The .ms file is
28 KB, the .PS.Z file is 56 KB.  A plaintext compilation (nroff) looks
a bit terrible since they're using boxes all over the place.
