I have the same modem.  Mine sits on com3: @3e8 IRQ 5.  FreeBSD is also 
set for IRQ5 for com3:  It uses 2e8 IRQ9 for com4:  To setup your comm 
program for use with your modem, just set the device file the comm 
program looks for to  cuaa0 for com1:  cuaa1 for com2:   cuaa2 for com3: 
cuaa3 for com4:
