It would probably be even easier to simply make this forms or
Q&A based (what's your IP address?  What's your ISPs phone number?
Any dialer prefix I should use?  CHAP, PAP or login script? ...)
but then we've been waiting for somebody to do this for, oh,
I'd say at least 2 years now. ;-)
