This looks like someone (jamesn@nix.what.net?) is piping email to sendmail -t (perhaps via UUCP, look at the Received: headers) instead of keeping the envelope To: intact, and thus is simply looping the email. This is someone on the cvs-all list, looping email back to cvs-committers (since committers is on the To: line). Presumably the mailing list loop avoidance is working for the cvs-all list.
