I use the same or a similar card.

In my kernel config file I have:

device pcm0 at isa? irq 10 drq 1 flags 0x0

On startup I see:

pcm1: <Yamaha OPL-SAx> at port 0x220-0x22f,0x530-0x537,0x388-0x38b,0x330-0x331,0x370-0x371 irq 5 drq 0,1 on isa0

It works great for me. I use 4.2, but have had this working back quite a few
versions. Also, I did not use any opl0 configuration options in the config
file. That is for not any reason in particular, but more because I got it
working without it.
