It sounds like you might be trying to use too high of a refresh rate.  I
didn't see it in your message, but if the left and right sides are switched
and there is a gap in the middle then that is probably the case.  Of course
unless you are running xdm or something you might not be able to tell if
the sides are switched or not.  You also might want to try a couple
of generic video modes (640x480x8 or 640x480x16) to make sure that it
is using the proper "driver" for your card.
