Simply add your ppp command to /etc/rc.local and it'll start
automatically.

It depends on the vendor, but in general you just have to install a DHCP
client and off you go.  The FreeBSD Projects page at
http://www.freebsd.org/projects/ has a link to a Cable Modem doc, I think.
