Implement code as src/lib/libthread_db/pthread/pthread_db_i386.c,
this code does conversion work between struct reg and struct
ucontext, used for M:N thread.
In kernel, make sure ptrace_single_step and ptrace_clear_single_step
work, they are in machdep.c, and used by M:N thread too.
