I've just looked further at this problem and realised that it was my
fault that this problem was occurring.  I had the GNU pth port
installed, which includes a pthread.h in /usr/local/include, with a:

#define fork __pthread_fork

When I remove the pth package, everything works as expected.  *sigh*
