I'm also using MSI 694D board.  It seems that you connected the hard drive
and the cd-drive to the 686B controller.  I had big problems last time I did
that (in SMP mode only).  So I tried moving the hard drive to the on-board
Promise controller and my problems went away.  Hope this helps!
