With a stock lpr/lpd, Firefox and xpdf work as expected.  My .xpdfrc is:
   psFile "| lpr -Plaser"

"laser" is the queue for my laser printer, defined in /etc/printcap.

If the real BSD lpr is being called, you should see errors in 
/var/log/lpd-errs.
